ABSTRACT

OBJECTIVE To study the effects of Dayuanyin decoction (DYY) on lung injury and gut microbiota in mice with RSV-Hanshi yufei syndrome. METHODS Thirty-six BALB/c mice were divided into the normal group (NC group), model group (MC group), positive control group (LBM group, 57.59 mg/kg ribavirin), DYY low-dose, medium-dose and high-dose groups (LDYY,MDYY,HDYY groups,1.67,3.34,6.68 g/kg,calculated by crude drug), with 6 mice in each group. Except for NC group, RSV-Hanshi yufei syndrome model was induced by “Hanshi modeling+RSV infection”; after the second cold and wet stimulation, RSV solution was dripped into the nasal cavity at 50% tissue culture infectious dose, once a day, for 3 consecutive days; each administration group was given corresponding solution intragastrically 4 hours after each nasal drip, once a day, for 5 consecutive days. The levels of motilin (MTL) and gastrin (GAS) in serum, the levels of IL-6 and IL-1β in supernatant of lung tissue, and the percentage of peripheral blood lymphocytes were all determined. The pathological changes of lungs were observed, and the changes of gut microbiota were analyzed. RESULTS Compared with MC group, the levels of MTL in serum (except for HDYY group), and the levels of IL-6 and IL-1β in supernatant of lung tissue were all decreased significantly in DYY groups, while the level of GAS (except for HDYY group) in serum was increased significantly (P<0.05 or P<0.01). The pathology of lung tissue was improved, the percentage of T lymphocytes in LDYY group and the percentages of T lymphocyte and B lymphocyte in MDYY group were all increased significantly (P<0.05 or P<0.01). Compared with the NC group, Bacteroidetes, Actinobacteria, Bacteroides, Alistipes, Bacteroidota, Bacteroides acidfaciens and Alloprevotella were expressed in high abundance, while Firmicutes, Proteobacteria, Lachnospiraceae NK4A136 group, Kineothrix and Clostridiales unclassified were expressed in low abundance. After the intervention of medium-dose DYY, the relative abundance of each microbiota tended to be adjusted back, with different species including Lachnospiraceae_UCG-Ruminococcus, etc. CONCLUSIONS Dayuanyin decoctioncan reduce the lung injury caused by RSV, the mechanism of which may be associated with relieving inflammation, regulating gastrointestinal hormone levels, the percentage of lymphocytes and the abundance of beneficial and harmful bacteria in the intestinal tract.

ABSTRACT

Objective:To investigate the predictive value of plasma atherosclerosis index (AIP) on metabolic associated fatty liver disease (MAFLD) in physical examination population. Methods:It was a cross-sectional study. Total of 97 076 people who completed physical examination in the Health Management Center of the First Affiliated Hospital of Anhui Medical University from January to December 2021 and met the integrity of the study were selected as study subjects. Of the subjects, 31 176 people who met the diagnostic criteria of MAFLD were set as the MAFLD group, and the other 65 900 people were set as the non-MAFLD group. Laboratory indexes, height, weight, blood pressure, liver ultrasound and other indicators in the two groups were collected, and the AIP was calculated. The t-test was used for measurement data and chi-square test was used for counting data to compare the differences between the two groups. Univariate and multivariate logistic regression were used to analyze the influencing factors of MAFLD. The two groups were grouped further according to gender and age, and the difference of AIP prediction efficiency in different groups was evaluated by receiver operating characteristic (ROC) curve. Results:The average age (47.3 years, Z=-31.734), male proportion (76.9%, χ2=7 837.54) and the average value of AIP (0.23, Z=-155.089) in MAFLD group were all higher than those in non-MAFLD group (all P<0.001). After stratified by age, gender, body mass index (BMI), hypertensive or not, hemoglobin A1c (HbA 1c), triglyceride (TG), fasting blood glucose (FBG), the difference of AIP between the two groups was still statistically significant (all P<0.001). Multifactorial regression analysis showed that alanine aminotransferase (ALT) ( OR=1.024), aspartate aminotransferase (AST) ( OR=0.974), serum creatinine (sCr) ( OR=0.975), serum uric acid ( OR=1.004), HbA 1c ( OR=1.231), hemoglobin (HB) ( OR=1.011), platelet(PLT) ( OR=1.002), FBG ( OR=1.131), BMI ( OR=1.419), AIP ( OR=11.318), systolic blood pressure ( OR=1.002), and diastolic blood pressure ( OR=1.012) were independent risk factors for MAFLD (all P<0.001). In the overall population, AIP had an area under the ROC curve (AUC) of 0.808, a cut-off value of 2.045, a sensitivity of 74.5% and a specificity of 72.4%; in the gender subgroup, the AUC was greater in women than in men (0.815 vs 0.764), and the cut-off values, sensitivities and specificities in the two groups were -0.044 vs 0.091, 75.6% vs 72.2%, 73.3% vs 67.6%, respectively; in the age sub-group, the largest AUC (0.848), cut-off value (0.034), sensitivity (79.1%) and specificity (75.3%) were found in the 18-44 years group; the differences were statistically significantin the ROC curve analysis of each group ( P<0.001). Conclusion:AIP is an independent risk factor for MAFLD, which has good predictive value for the occurrence of the disease, and has better predictive effect in women and young groups (18-44 years old).

ABSTRACT

Objective:To evaluate the standardized residency training of obstetrics and gynaecology in Shengjing Hospital of China Medical University with Milestones evaluation system of obstetrics and gynecology, and to evaluate the feedback effect of the system. Methods:An improved Milestones evaluation form was developed to evaluate the self-evaluation and clinical tutor's scores, collect the results and make a statistical analysis of the 1-3 years standardized training residents of the Shengjing Hospital of China Medical University in September 2019. GraphPad Prism 8 software was used for data collection and statistical analysis, and Box-Plots software was used for drawing analysist. Descriptive statistics method was used for the general situation of the research objects. After the students' self-evaluation and tutor's scores passed the normal distribution and variance homogeneity test, paired t test was performed, and the scores of residents from different sources were analyzed by variance analysis. Results:A total of 164 residents of obstetrics and gynaecology were evaluated, and there were significant differences between self-evaluation and teacher's evaluation in medical knowledge, learning ability and system work ( P<0.05). The competency scores of the first year residents ranged from 3.003 to 4.556, and at the end of the course they increased to 4.921 to 5.974, showing statistically significant differences in each item compared with the first year ( P<0.001). After three years of training, residents from different sources showed significant improvement ( P<0.001). Conclusion:The Milestones evaluation system of the obstetrics and gynecology can be used for the training assessment and feedback of the residents in obstetrics and gynecology specialty. The self-assessment of residents helps to understand the level of self-assessment, and the teacher's evaluation helps to correct the bias generated by self-assessment. The systematic and staged Milestones evaluation system can instruct the clinical tutors to instruct the residents in accordance with their aptitude, and give feedback on the training effect of the residency training base.

ABSTRACT

OBJECTIVE: To study the correlation of tacrolimus concentrations among transplant patients’ whole blood, plasma and blood cells, analyze the effects of transplant types and ages on the their correlation, and to provide reference for rational drug use in clinic. METHODS: Totally 20 patients receiving tacrolimus anti-rejection therapy after transplantation and undergoing therapeutic drug monitoring (TDM) were randomly selected. According to the type of transplantation, they were divided into renal transplantation group and lung transplantation group (10 cases for each group).  According to age, they were divided into three groups: 20-40 years old group, 41-60 years old group and 61-80 years old group (4, 9, 7 cases for each group). Their residual blood after TDM was collected. Chemiluminescence microparticle immuno assay  (CMIA) was used to detect the concentration of tacrolimus in whole blood. UPLC-MS/MS was used to measure the concentrations of tacrolimus in plasma and blood cells. Pairs plots and Spearman rank correlation analysis were used to analyze the correlation of tacrolimus between whole blood and plasma, between whole blood and blood cells, between plasma and blood cells as well as the effects of transplant types and ages on tacrolimus concentrations among tansplant patient’s whole blood, plasma and blood cells. RESULTS: The correlation of tacrolimus concentrations in whole blood and plasma (r=0.623,P<0.01) was slightly stronger than that of whole blood and blood cells (r=0.591, P<0.01); while  the correlation of tacrolimus concentration in plasma and blood cells was relatively weak (r=0.497,P<0.05). Transplant type and age had an effect on the correlation of tacrolimus concentrations among patients’ whole blood, plasma, blood cells. The correlation of tacrolimus concen- tration in whole blood, blood cells and plasma in renal transplantation group was also weak (all r<0.5), and was weaker than that in lung transplantation group. The correlation of tacrolimus concentration among whole blood, plasma and blood cells was weak in patients of aged 20-40 years old group (all r<0.3), and was weaker than that of patients of aged 41-60 years old group and 61-80 years old group. CONCLUSION: Post-transplantation patients’ tacrolimus concentrations in whole blood, plasma and blood cell have a weak correlation. Rejections and adverse effects should be monitored in these patients, especially those renal transplantation patients or those patients aged 20 to 40.

ABSTRACT

Objective To compare the effect of ultrasound measurements of the distance from skin to vocal folds (DSV), neck diameter (d) and the ratio among the two measurements (R) used to predict difficult laryngoscopy. Methods Seventy-two toddelers scheduled to undergo general anesthesia, aged ≤ 3 years, falling into ASA physical status Ⅰ or Ⅱ, were included and categorized as having easy (n = 48, grades Ⅰ and Ⅱ) or difficult (n = 24, grades Ⅲ and Ⅳ) laryngoscopy based on the criteria of Cormack and Lehane. When children were ventilated by mask, we measured DSV by ultrasound; obtained d and R. Receiver operating characteristic (ROC) curve analysis was used to determine the DSV, d and R in predicting difficult laryngoscopy. The value of cutoff was obtained by maximizing the Youden's index. Results DSV was significantly shorter in difficult laryngoscopy group (P < 0.05). The areas under the ROC curve of R was 0.807, and the cutoff value was 0.090. the sensitivity was 70.83%, the specificity was 83.33% Conclusion The ratio of distance from skin to vocal folds to neck diameter is a good predictor of difficult laryngoscopy in children under 3 years old.

ABSTRACT

OBJECTIVE:To investigate the effects of blood concentration monitoring of cylosporin A(CsA) in patients with nephrotic syndrome(NS)on efficacy and safety. METHODS:The medical records of 154 NS patients receiving CsA and blood concentration monitoring in nephrology department of China-Japan Friendship Hospital during Jan. 2014-Aug. 2017 were analyzed retrospectively. The results of blood concentration monitoring in 63 adult inpatients with refractory NS receiving CsA for the first time within 6 months of initial treatment were analyzed statistically. The relationship of blood concentration monitoring with efficacy and safety was analyzed. RESULTS:The blood concentration of CsA in 154 patients were monitored for 512 times with an average of 3.32 times/person,and average blood concentration was(125.98±105.13)ng/mL. The patients with blood concentration of CsA<100 ng/mL accounted for 44.14%. There was no statistical significance in average monitoring times or average blood concentration between male and female, average blood concentration of CsA among different age groups (P>0.05). The blood concentration was monitored for 237 times in 63 adult inpatients with refractory NS receiving CsA for the first time within 6 months of initial treatment(induction period). Average blood concentration of effective group were significantly higher than ineffective group;the proportion of effective group with blood concentration<100 ng/mL was significantly lower than that of ineffective group, with statistical significance (P<0.05). Among 63 patients,17 patients suffered from ADR (the incidence of ADR was 26.98%). The average blood concentrations of ADR patients were significantly higher than those without ADR;the monitoring times of ADR patients with blood concentration>150 ng/mL was significantly higher than those without ADR,with statistical significance(P<0.05). There was no statistical significance in the incidence of ADR between effective group and ineffective group (P>0.05). Among effective group, there was no statistically significance in average blood concentration between ADR patients and patients without ADR(P>0.05);the monitoring times of ADR patients with blood concentration>150 ng/mL was significantly higher than patients without ADR,with statistical significance(P<0.05). With the increase of monitoring times, the incidence of ADR decreased gradually. There was no statistical significance in the incidence of ADR among patients who were monitored for different times (P>0.05). CONCLUSIONS:The pharmacokinetics of CsA varies in different patients and many factors affect its blood concentration. The changes of blood concentration affect the efficacy and safety of CsA. It is difficult to determine the dosage of CsA based on experience in the treatment of NS with CsA. Great importance should be attached to blood concentration monitoring of CsA and the implementation of individualized dosage regimen based monitoring results so as to improve therapeutic efficacy and reduce the occurrence of ADR.

ABSTRACT

<p><b>OBJECTIVE</b>To evaluate three different methods for controlling presacral massive bleeding during pelvic operations.</p><p><b>METHODS</b>Clinical data of 11 patients with presacral massive bleeding during pelvic operation at The Sixth Affiliated Hospital of Sun Yat-sen University and 157 Branch Hospital of Guangzhou General Hospital of Guangzhou Military Command from January 2001 to January 2016 were analyzed retrospectively. Hemostasis methods for presacral massive bleeding during operation included gauze packing (whole pressure), drawing pin (local pressure) and absorbable gauze (absorbable gauze was adhered to bleeding position with medical glue after local pressure). Efficacy of these 3 methods for controlling bleeding was evaluated and compared.</p><p><b>RESULTS</b>Ten patients were male and 1 was female with average age of 65.2 (40 to 79) years old. Eight cases were rectal cancer, 2 were presacral malignancies and 1 was rectal benign lesion. Bleeding volume during operation was 300 to 2 500 (median 800) ml. From 2001 to 2012, 4 cases received gauze packing, of whom, 3 cases were scheduled Dixon resection before operation and then had to be referred to Hartman resection; 3 cases died of systemic failure due to postoperative chronic errhysis and infection, and 1 underwent re-operation. At the same time from 2001 to 2012, 5 cases received drawing pin, of whom, bleeding of 3 cases was successfully controlled and Dixon resection was completed. In other 2 cases with hemostasis failure, 1 case underwent re-operation following the use of gauze packing, and another 1 case received absorbable gauze hemostasis. All the 5 patients were healing. From 2013 to 2016, 2 cases completed scheduled anterior resection of rectum after successful hemostasis with absorbable gauze and were healing and discharged.</p><p><b>CONCLUSIONS</b>Gauze packing hemostasis is a basic method for controlling presacral massive bleeding. Drawing pin and absorbable gauze hemostasis are more precise and may avoid the change of surgical procedure. But drawing pin has the possibility of hemostasis failure. Absorbable gauze hemostasis with medical adhesive is effective, simple and fast.</p>

ABSTRACT

Porcine epidemic diarrhea virus (PEDV) inhibits the host typeⅠinterferon and cellular antiviral response, but its inhibition mechanism is unclear, and the roles of PEDV nonstructural proteins in regulating typeⅠinterferon responses have been seldom studied. To study the effect of nsp1 on typeⅠinterferon response, nsp1 gene was cloned into a eukaryotic expression vector pCAGGS. The expression of nsp1 in transfected cells was determined by Western blot and indirect immunofluorescence assay. The effects of nsp1 on the induction of typeⅠinterferon were evaluated by dual luciferase reporter gene assay, ELISA and VSV bioassay. Western blot and indirect immunofluorescence assay showed that nsp1 was highly expressed in transfected cells and PEDV-infected cells. Dual luciferase reporter gene assay results indicated that nsp1 strongly inhibited the IFN-β promoter activity, and the inhibitory effect was nsp1 dose-dependent. ELISA results showed that nsp1 significantly inhibited the expression of IFN-β in protein level. And VSV replication-inhibition bioassay revealed that nsp1 significantly inhibited typeⅠIFN antiviral activities induced by poly(I:C). Our results implied that nsp1 was a highly conserved protein of PEDV and exhibited antagonistic function on interferon promoter activity. The results have laid a foundation for further understanding the immune evasion mechanism of PEDV and for developing new effective vaccine against PEDV.

ABSTRACT

<p><b>OBJECTIVE</b>To compare fecal microbiota and metabolites between colorectal cancer (CRC) patients and healthy population.</p><p><b>METHODS</b>Feces from fifteen CRC patients and twelve normal people were analyzed by using pyrosequencing and gas chromatography mass spectrometry(GC/MS).</p><p><b>RESULTS</b>There were no significant differences in the overall microbial community structure associated with the disease state, but 18 bacterial genera were underrepresented or overrepresented in the CRC samples. GC-MS profiling revealed higher concentrations for 9 kinds of amino acids and metabolites of short-chain fatty acids, lower concentrations for 3 kinds of unsaturated fatty acids and 2 kinds of glycerin and ursodeoxycholic acid in stool samples from CRC patients. Correlative analysis between the combined datasets revealed some potential relationships between stool metabolites and certain bacterial species.</p><p><b>CONCLUSIONS</b>There are significant differences in fecal metabolites and the relative abundance of certain types of bacteria between CRC patients and healthy people, which can provide insight into microbial functions occurring in a cancer environment and will help direct future mechanism studies.</p>

ABSTRACT

Objective To study the immunophenotype and chromosome karyotype of 57 adult patients with newly?diagnostic acute lymphoblastic leukemia(ALL). Methods The immunophenotype and chromosome karyotype of 57 adult patients with newly?diagnostic ALL were determined by using flow cytometer and karyotyping, and then the clinical characteristics and significance were analyzed. Results In patients with B cell acute lymphoblastic leukemia(B?ALL),the expression of CD19 and CD22 were significantly higher than CD10 and CD20(P<0.05). The expression of CD7 and CD3 were much higher in patients with T cell acute lymphoblastic leukemia(T?ALL). As for the early antigen, patients of all subtypes of ALL showed high expression rate of CD34 and CD38. In addition, the HLA?DR was only detected in B?ALL. There was no difference in comparison with clinical characteristics, total rate of CR and rate of relapse between ALL with and without myeloid antigen expression. The expression rate of CD34 and HLA?DR was higher in My+?ALL(P<0.05). The relapse rate was much higher in these patients with abnormal karyotype than other pa?tients(P<0.05). Patients with high?risk karyotype showed higher expression rates of CD20 and CD3 than those with standard?risk. Conclusion The immunophenotypes and chromosome karyotypes show obviously heterogeneous features, and the patients with abnormal karyotypes have higher relapse rate. These above mentioned methods are applicable for the diagnosis and individualized treatment of ALL patients.

ABSTRACT

Electrochemiluminescence was used to determine thyroid function and thyroid autoantibodies in 140 pregnant women, who were then divided into normal group (n =117) and subclinical hypothyroidism group (n =23) based on the thyroid function. The urine iodine level in the pregnant women was detected by arsenic cerium catalytic spectrophotometric method. The awareness of past history of thyroid disease among the subjects with thyroid dysfunctions were investigated. The results showed that the prevalences of iodine deficiency were 50% and 57% in the normal group and the subclinical hypothyroidism group, respectively. The state of iodine level was not related to thyroid function. The levels of serum thyroid peroxidase antibody (TPOAb) and thyroglobulin antibody were markedly related to serum TSH(P<0.01),so was the level of serum TPOAb related to serum FT4 (P<0.05).Among the subclinical hypothyroidism women,70% did not undergo thyroid function and thyroid autoantibodies screening before pregnancy,8.7% denied past history of thyroid disease, and 21.7% suffered from hypothyroidism before pregnancy. Therefore,we advocate the screening of urine iodine and thyroid autoantibodies before or during the first trimester of pregnancy, aiming to correct iodine deficiency, avoid supplementing too much iodine, improve the outcome of perinatal stage, and reduce all the negative effects on the offsprings.
